To your brain it logically sees the situation as being OK and pleasant and sends a message to your subconscious to which it looks up the programmed response and says "OK, time to smile."
In the case of Claustrophobia, you enter a crowded room and the logical part of your brain sees the room, sees the people and sends it along to your subconscious. Your subconscious looks up it's programed response and says "OK, time to kick in the Claustrophobia Symptoms". That is when the panic and fear start to set in. Literally there is nothing you can do about it until the programed response is changed.
